Ingrown Toenail Matrixectomy
Recurring ingrown toenails cause significant pain and can lead to serious infection. A matrixectomy is a simple, permanent in-office procedure that eliminates the problem once and for all — with minimal downtime and immediate relief.
What Is a Matrixectomy?
A matrixectomy is a procedure in which a portion of the toenail matrix (the tissue that produces the nail) is permanently destroyed, preventing the problematic nail border from regrowing. It is the definitive treatment for chronic, recurring ingrown toenails.
The procedure is performed in the office under local anesthesia. The offending nail border is removed and the matrix is treated with a chemical agent (phenol) or laser to prevent regrowth. The procedure takes approximately 15–30 minutes and patients walk out of the office immediately afterward.
Unlike trimming the nail at home or having it temporarily removed, a matrixectomy provides permanent relief — the ingrown nail will not return.

Signs You May Need a Matrixectomy
Symptoms of an Ingrown Toenail
- • Pain and tenderness along the side of the toenail
- • Redness and swelling around the nail border
- • Infection (pus, increased warmth, odor)
- • Skin growing over the nail edge
- • Recurring pain despite conservative trimming
- • Difficulty wearing shoes or walking
Who Should Consider a Matrixectomy?
- • Patients with recurring ingrown toenails despite conservative care
- • Diabetic patients (high infection risk requires definitive treatment)
- • Patients with nail deformities that predispose to ingrown nails
- • Patients who have had multiple temporary nail removals
- • Anyone who wants a permanent solution rather than ongoing management

What to Expect
Local anesthesia is administered to completely numb the toe. You will feel no pain during the procedure.
The offending nail border is removed and the matrix is treated with phenol to prevent regrowth. The procedure takes 15–30 minutes.
You walk out of the office immediately. The toe is bandaged and you can wear open-toed shoes. Most patients return to normal activities within 1–2 days.
The treated area heals over 2–4 weeks. Dr. Bregman may incorporate laser therapy to accelerate healing and reduce post-procedure discomfort.
